This role plays a vital part in supporting clients and caregivers within our Private Duty Care and Consumer Directed Services (CDS) programs.If you enjoy a fast-paced environment, value relationship-building, and are ready to make a real difference in your community, we encourage you to apply today.What You'll Do: Serve as a professional representative of Phoenix with clients, caregivers, and the community Manage and process intake paperwork for new Private Duty Care clients Coordinate start-of-care logistics in collaboration with the Care Coordinator Support ongoing monthly case management duties for CDS Assist with electronic visit verification (EVV) compliance and visit accuracy Maintain up-to-date, clear, and accurate client records Conduct frequent in-home visits with clients across multiple counties Prepare and submit timely documentation and reports per policy guidelines Participate in community outreach and marketing events and initiatives Support general clerical tasks and assist with resolving client or caregiver concerns Attend in-services, staff meetings, and quality assurance activities What You Bring: Minimum age: 21 years High school diploma or equivalent required Supervisory experience preferred Experience in an office setting required Ability to work independently while staying organized and detail-oriented Strong communication skills with the ability to conduct client interviews professionally Ability to manage multiple tasks in a fast-paced environment Previous home care experience is highly preferred Physical Requirements: Frequent sitting, standing, walking, and manual dexterity Occasional lifting up to 50 lbs and pushing/pulling over 100 lbs Occasional reaching above shoulder level Must be able to drive for business-related travel Why Join Phoenix?
